PARTICIPATION POLICIES
I, parent/guardian, of the named candidate for the Excel Youth Academy Golf and S.T.E.A.M program, approve participation in any and all activities and educational sessions. If necessary, I assume all risks and hazards incidental to such participation, including transportation; if necessary. I do hereby waive, release, absolve, indemnify and agree to hold harmless the organizer, its supervisors, participants, and persons transporting the players to and from activities for any claim arising out of any injury to the player except to the extent and in the amount covered by an accident and/or liability insurance held by the organizer. I also grant permission to managing personnel or other league representatives to authorize and obtain medical care from any licensed physician, hospital or medical clinic should the player become ill or injured, while participating in program activities, away from home, or at any other time when neither parent nor guardian is available to grant authorization for emergency treatment.

Financial Responsibility

In order to begin an official EYA membership, I agree to pay the membership fee for the current semester (two semesters per EYA year) by credit card or EFT (electronic funds transfer from my savings/checking account). My Checking/Savings/Credit Card account will not be charged automatically for renewal. Current members will receive a 30 day notice to renew membership before expiration to continue benefits for the following semester. I understand there are no refunds given. It is my responsibility to check my monthly bank or credit card statement and report any discrepancies within 30 days to the Excel Youth Academy. Should any membership draft/charge not be honored by my financial institution for any reason, I am still responsible for that payment. If I wish to cancel my membership, I may do so at any time with the understanding of forfeiting my current membership fee. The Excel Youth Academy Reserves the right to increase membership fees as necessary and will notify me in writing at the address I have provided at least 30 days prior to the increase roll-out for the following semester.
Membership Waiver

In consideration for being permitted to utilize equipment, golf course facilities, services and and programs of The Excel Youth Academy, Inc. (“EYA”), I, on behalf of myself and my heirs, personal representatives, assigns and next-of-kin, do hereby agree to the following: The activities that I will be engaging in while I am in or upon the premises of an EYA clinic or class, using any of its facilities, services or equipment, or participating in any EYA program or activity I hereby accept full responsibility for, and risk of, any injury to myself or loss or damage to my property that may occur as a result thereof. In the event of injury, I authorize the Releases to provide or cause to provide such medical care and treatment to me as necessary and appropriate. I understand that I am solely responsible for all costs incurred for such medical care or treatment.

I give my permission to the EYA to use indefinitely, without limitation or obligation, photographs, film footage, or tape recordings which may include my image or voice to promote or interpret EYA programs and activities.
Cancellation Policy
If I wish to terminate my membership, I may do so at any time by emailing contactus@excelyouthacademy.com. My membership will remain in effect until I receive a cancellation confirmation email. No refunds will be issued if the registered participant attends any Exposure Golf & S.T.E.A.M clinics. A Membership is valid for one year.
